1. Birth certificate should be issued by a hospital or clinic; DNA analysis report should be issued by a medical centre. 2. The parents’ household registration form, applicant’s ID card and personal seal (or signature, and with the date of marriage of the parents). 3. For the one who is born abroad, the parents should apply to the Immigration Office for the resident certificate, and then apply for the household registration. 4. The birth registration for the abandoned child or orphan should enclose the testimony of the police station and a photo of the baby. The baby without a name should be named by the carer or the child welfare service. 5. The child from a marriage should follow father’s surname. If the child is going to use mother’s surname, a household certificate and an agreement should be enclosed to prove that the mother has no brothers. 6. The director of the Household Registration office has the right to register and to name the child if the parents are noticed and still do not register after the due date. 7. The child who is born in the country should register based on the Household Registration Law after the revision of the Nationality Law on February 9th, 2000. 8. For the one dies in the foreign countries, the death certificate should be issued by the local hospital and inspected by the Embassy of the Rep of China. The non-Chinese documents should be translated into Chinese and inspected by the Bureau of Consular Affairs or police bureau. 4. Death announcement for the one has been missing for 7 years (3 years for the one over 80, 1 year for the one missing in a disaster) should be applied by the relatives or the prosecutors to the district court with court’s judgment. 5. For the one dies in Mainland China, the death certificate issued by the local hospital should be inspected by the Straits Exchange Foundation. 6. For those who die for years without the death certificate issued by the doctor or the burial permit, the death registration should be applied with the testimony from the police and the statement of the witness. Notes:1. For those die in a disaster, with an unknown identity or nationality, the police station should report to the local household registration office after the inspection. 2. For the soldier who dies in service, the death registration should be applied with the death report issued by the military agencies. 3. For moving into the house owned by the applicant, applicant’s spouse, direct blood relative, affinity, or other people with a free use, a document of the ownership of the house or the latest house tax statement or a contract of sale in the latest six months or other documents proving the ownership should be enclosed. 2. For moving into the house rented by the applicant, applicant’s spouse, direct blood relative or the affinity, and the lease contract notarized by court should be enclosed. If it is not notarized by court, the document of the ownership or the latest house tax statement or a contract of sale in the latest six months of other documents proving the ownership of the leaser should be enclosed. For some special cases, the original copy of the lease contract should be enclosed. The photo copies of the documents of the ownership should be signed by the owner of the house. 3. For moving into a factory, store, temple, office, school or other public place, an agreement of the household head or the place keeper should be enclosed. 4. A bill of water and electricity or gas in the latest six months paid by the applicant, the applicant’s spouse, direct blood relatives or affinity should be enclosed. 5. Adoption termination registration |

1. An agreement of adoption termination or court’s final judgment (if the incident happens in a foreign country, the document should be inspected by the Embassy and translated into Chinese for the inspection of the court or the Embassy). 2. After the adoptive parents die, the adopted children can apply the adoption termination to court if the adopted children are incapable of earning a livelihood. A Court’s adoption termination and the Court’s final judgment should be enclosed. 3. Household registry form, the ID cards of both the adopter and the adopted (only for those who have the ID card), the applicant’s personal seal or signature. 4. If the adopted children have an ID card, 3 2.5” head-shot photos in the latest 2 months (either black-and-white or colour) and 50 NTD should be enclosed for the re-issuing of the ID. |

1、Both parties’ ID cards, personal seals (or signatures), household registry forms, a colour head-shot photo within a year and the marriage agreement. 2、Both parties must register in the specific household registration office in person. 3、The marriage of the minors should enclose an agreement from the Legal agents. 4、Other documents for the ID proving: (1). If the marriage is held in a foreign country, an authorization letter, the authorized agent’s (listed on the authorization letter) ID card and personal seal (or signature) is required. The authorization letter must be inspected by the Embassy or the organization assigned by the Executive Yuan in Hong Kong or Macau. (2). If either party of the marriage once had or never has a household registry in Taiwan, the passport and the alien residence certificate issued by the Immigration office must be enclosed. 5、Other documents for proving the marriage: (1). If the marriage is held in a foreign country, a marriage certificate or other documents of marriage issued by the local government and its Chinese translation with the seal of “Comply with the Law of conduct” should be enclosed. All documents must be inspected by the Embassy or the organization assigned by the Executive Yuan in Hong Kong or Macau. (2). If one of the parties is not a citizen of Taiwan, a copy of Chinese name registration statement by foreign spouses, documents of marriage status and its Chinese translation inspected by the Embassy should be enclosed. (3). The Chinese translation of the marriage certificate in a foreign country should be notarized by the Embassy or a notary in Taiwan. (4). For a person who marries the people from Mainland China, a marriage certificate and entry permit issued by the Immigration Office with the seal of “Interviewed for the marriage registration” should be enclosed. The documents made from China, Hong Kong or Macau should be inspected by the organizations assigned by the Executive Yuan. |

Divorce registration |

1. Divorce agreement, court’s judgment and final judgment. 2. If the divorce is held in a foreign country, the certificate must be inspected by the Embassy or the organizations acknowledged by the government. The certificate must be translated into Chinese and be inspected by the Bureau of Consular Affairs or Court. The authorization letter should also be inspected. 3. Divorce (including the authorization letter) the people from China, related documents must be inspected by the Straits Exchange Foundation. The final judgment of divorce should also be acknowledged by the courts in Taiwan. 4. Both parties must come to the office in person. The effective date of divorce is the date of registration. 5. Both parties’ household registry forms, ID cards, personal seals (or signatures). 6. This registration requires the re-issuing of ID card. 1. In the same office, organization, group, or school there are people having the same identical name. A proving document issued by the employer or school should be enclosed. 2. The same identical name with a direct blood relative in three degrees. A household certificate should be enclosed. 3. There are people having the same identical name and living in the same county (city) over six months. A household certificate of the people having the same name should be enclosed. 4. There are people having the same identical name in an evaluation in an organization. A notification from the evaluating agency should be enclosed. 5. Sharing the same identical name with the wanted person. A document or a notification about the wanted person should be enclosed. 6. The name contains a disagreeable or offensive implication or with other special reason which is acknowledged by the administrative agencies. Notes: The application should enclose a copy of the household certificate of the present household, ID card, personal seal (or signature), household registry form. The minors who apply for the name change should be accompanied by the both parents. If either of the parents is unable to accompany, an agreement should be enclosed. If the applicant has a spouse or direct blood relatives, 3 2” head-shot photos (black-and-white or colour) taken in the latest 2 months and 50 NTD should be handed in for the re-issuing of the ID card. The applicant (if the applicants are the minors, it should be applied by the legal agent) should prepare his/her and the relatives’ (spouse, parents, adoptive parents, children or household head) passports or other documents with the English name on it. The needed page of the passport and the other documents must be photocopied. The applicant’s ID card and personal seal and the application fee are also required. Documents with the English name on it include: (1) The English identity proving documents issued by the government of Taiwan (2) The English identity proving documents issued by the foreign government. (3) The English birth certificate issued by a hospital. (4) The diploma with the English name issued by schools. (5) An English name proving document issued by the overseas organizations registered to the government. 2. If the applicant is unable to provide a passport or any documents with the English name on it, the applicant should fill in the English name based on the Chinese phonetic translation guidelines. Authorized application 1. The applicant should fill in the authorization letter and sign (or seal). The authorized agent should prepare his/her ID card, personal seal and the authorization letter for the application. 2.
